- Well-baby and well-child visits, ages birth to 22
- Blood pressure, diabetes, and cholesterol tests
- Annual cancer screenings for certain age groups

- Routine hearing exams every 24 months

Q: How do I receive medical care?
A: Simply select an in-network health care according to your plan and call to make an appointment.
When at the appointment, present your medical ID information using your app or on your card. You may still receive coverage without your physical ID card as well, by showing your electronic copy of your ID card on your Elevate by Nextep mobile app!
After receiving care, simply pay the health care provider according to your plan.
Aetna members: To find doctors that are part of your health team, visit aetna.com/docfind and find the whole health plan for your region.
Q: Will I receive an ID card?
A: You will receive new ID cards for each participant at your home address. Enrollments processed after your designated online enrollment period may result in ID cards arriving later, even though coverage will be effective.
To access your ID cards, log in at employee.nextep.com or on your Elevate by Nextep mobile app, then click on Benefits. Simply tap View Card to pull up an electronic copy of your ID card!
Q: What is a PPO plan?
A: Nextep's flex plans offer the affordability of a preferred provider organization (PPO) plan, but with a wider, more flexible network. These plans feature lower deductibles, perfect for families or individuals who would be high utilizers of the benefit.

Q: What is an HNO?
A: Health network option (HNO) plans offer coverage at lower costs. Get the same rich benefits you enjoy from PPO-style plans, including preventative care and prescription drugs. Since these plans are region-specific, it is very important to stay in-network.

Q: What is an HDHP?
A: High-deductible health plans (HDHPs) have higher deductibles and lower costs, allowing participants to put the extra cost savings into a health savings account (HSA).

Q: What is an EPO?
A: An EPO (Exclusive Provider Organization) health plan offers coverage for medical services when you use doctors, specialists, or hospitals within the plan's network. It's like having a preferred list of healthcare providers. While you don't need a referral to see specialists, going outside the network usually means paying the full cost, except in emergencies.
An EPO plan might be a good choice for you if:
- You prefer to manage your own healthcare without needing referrals from a primary care physician.
- You're comfortable choosing healthcare providers from a specific network.

A: With a health maintenance organization (HMO) plan, you must choose a primary care physician (PCP) from a network of local health care providers who will refer you to in-network specialists or hospitals when necessary. All care is coordinated through the PCP at a significant savings.
