Health Plan Features
Our medical plans cover in-network preventative services 100% with the deductible waived. Depending on age, members have access to all of these benefits, regardless of which plan they select:
- Annual routine physical exams for adults
- Routine gynecological exams
- In-network prenatal maternity office visits
- Well-baby and well-child visits, ages birth to 22
- Blood pressure, diabetes, and cholesterol tests
- Annual cancer screenings for certain age groups
- Flu and pneumonia shots, plus vaccinations
- Counseling and vaccines for healthy pregnancies
- Routine hearing exams every 24 months
- Counseling on topics such as smoking, weight, depression, and alcohol.

Medical Plan Terminology
New to benefits? Before you dive into the specific plan types, watch this quick guide to understand common words you'll see: premium, deductible, copay, coinsurance, and out-of-pocket maximum.
PPO Plans
With a Preferred Provider Organization (PPO) plan, you can go to any medical provider, but you'll pay less when you stay in-network. PPOs often feature lower deductibles.
EPO Plans
An Exclusive Provider Organization (EPO) plan covers your medical care only if you use doctors and hospitals within its specific network, except in a true emergency.
POS/Flex Plans
A Point of Sale (POS, or "Flex") plan lets you receive care from any doctor in or out of the network, but costs are lower when you choose an in-network provider.
HDHP Plans
A High-Deductible Health Plan (HDHP) offers lower monthly premiums and the eligibility to hold an HSA. To offset the savings, they often have a higher deductible.
HNO Plans
The Health Network Organization (HNO) plan covers your care only when you use in-network of doctors, but lets you skip choosing a primary care physician.
HMO
A Health Maintenance Organization (HMO) plan keeps your costs low by requiring you to get in-network care and coordinate through a primary care physician.

To access your ID cards, log in at employee.nextep.com or on your Elevate by Nextep™ mobile app, then click on Benefits. Simply tap View Card to pull up an electronic copy of your ID card!
