Our vision plans cover a wide array of vision services and materials, including routine eye exams, your choice of glasses or contact lenses, fittings, plus additional savings on prescription sunglasses and laser vision correction.

You have the option between the Core plan, offering great benefits at a great price, or the buy-up Enhanced program, offering the same rich perks you are used to.

When it's time to enroll, you can view pricing for all plan options right in your Elevate by Nextep™ account or mobile app. As you click through the enrollment screens, a running tally will track and display your monthly costs in real time.

You can go to any licensed eye care professional or choose from the thousands of ophthalmologists, optometrists, and opticians at private practices or popular retail locations such as Pearle Vision, Visionworks, and more.

For additional convenience, a special service arrangement with WalMart, Sam's Club, and Costco makes it easy for you to use your benefits even though they are out-of-network. They check your eligibility and process claims so there is less paperwork for you.

When at the appointment, use the employee's social security number as the member ID. Regardless of which family member is receiving care, use the policy-holding employee's SSN.

No ID card is needed for dental or vision plans.

Simply use the employee's social security number as the member ID when making your appointment. If you wish, though, you can access and show your ID card right from your Elevate by Nextep™ app on your mobile phone!

To access your ID card, log in to your Elevate by Nextep™ account or mobile app, then click on Benefits. Simply tap View Card to pull up an electronic copy of your ID card.

To locate an in-network provider, log in to your Elevate by Nextep™ account or mobile app and click the Vision card to access the carrier site with single sign-in.

Members should verify their providers are currently in MetLife's network when making appointments.

Out-of-network providers are not bound to the negotiated in-network cost, but our plan does allow generous reimbursements for select out-of-network services. To submit a claim for reimbursement of out-of-pocket costs, visit your Vision info in your Elevate by Nextep™ account or mobile app.

Nope. You and your family members each have the flexibility to visit the providers of your choice.

Yes. Check with your provider to see what their policy is for filling another provider’s prescription. Keep in mind, your plan only covers glasses OR contact lenses once per frequency, so check plan details.
