Our vision plans cover a wide array of vision services and materials, including routine eye exams, your choice of glasses or contact lenses, fittings, plus additional savings on prescription sunglasses and laser vision correction.
You have the option between the Core plan, offering great benefits at a great price, or the buy-up Enhanced program, offering the same rich perks you are used to.

- Annual eye exams covered 100% after $10 copay
- Choice of eyeglasses or contact lenses, covered 100% after copay, up to a certain allowance
- Savings on lens enhancements, including UV-resistant coating and progressive lenses
- 20% savings on prescription sunglasses
- Discounts on laser eye surgery, including LASIK and PRK
- Benefits can be used once per rolling 12-month year, as opposed to calendar year
- Core plan offers lower cost, with glasses allowance every 24 months

For additional convenience, a special service arrangement with WalMart, Sam's Club, and Costco makes it easy for you to use your benefits even though they are out-of-network. They check your eligibility and process claims so there is less paperwork for you.
When at the appointment, use the employee's social security number as the member ID. Regardless of which family member is receiving care, use the policy-holding employee's SSN.
Simply use the employee's social security number as the member ID when making your appointment. If you wish, though, you can access and show your ID card right from your Elevate by Nextep™ app on your mobile phone!
To access your ID card, log in to your Elevate by Nextep™ account or mobile app, then click on Benefits. Simply tap View Card to pull up an electronic copy of your ID card.

Members should verify their providers are currently in MetLife's network when making appointments.
Claim form
You can also log in to your Elevate by Nextep™ account or mobile app and click the Vision card to access the carrier site with single sign-in, and submit your claim online in your Vision account.
