Offered through a partnership between Nextep and MetLife, our dental plan offers four levels of coverage to meet your needs, from basic services all the way up to orthodonia, crowns, implants, and dentures.

All four levels of coverage include preventative services covered at 100%, including teeth cleaning, certain routine x-rays, and checkups. See plan summaries for details. All dental plans feature a low annual deductible of $50 individual or $150 family coverage.

Q: How do I receive dental care?

A: Simply select an in-network dentist according to your plan and call to make an appointment.

When at the appointment, use the employee's social security number as the member ID.

Regardless of which family member is receiving care, use the policy-holding employee's SSN.

After receiving care, simply pay the dentist according to your plan.

Q: Will I receive an ID card?

A: No ID card is needed for dental or vision plans.

Simply use the employee's social security number as the member ID when making your appointment. You can also access and show your ID card right from your Elevate by Nextep app on your mobile phone!

To access your ID card, log in at employee.nextep.com or on your Elevate by Nextep mobile app, then click on Benefits. Simply tap View Card to pull up an electronic copy of your ID card!

Q: How do I find an in-network provider?

A: To locate an in-network provider, log in to your account at employee.nextep.com, go to Benefits, and click Dental. Or, call MetLife at 1-800-GET-MET8. Members should verify their providers are currently in MetLife's network when making appointments.

If you're not yet a member and would like to preview the thousands of in-network general dentists and specialists to choose from nationwide, look for a list of participating dentists online at metlife.com/dental.

Q: How do I get out-of-network care?

A: Out-of-network providers are not bound to the negotiated in-network cost, but our plan does allow generous reimbursements for select out-of-network services. To submit a claim for reimbursement of out-of-pocket costs, visit your Nextep Account.

You are free to select any dentist, but we highly recommend choosing in-network dentists for maximum cost-management. In-network dentists accept negotiated fees for covered services; these fees are typically 15 - 45% below the average.
